[Autoerotic fatalities in Greater Dusseldorf]. / Autoerotische Todesfälle im Grossraum Düsseldorf.

Autoerotic fatalities in the Greater Dusseldorf area correspond to the relevant medicolegal literature. Our results included exclusively young to middle-aged, usually single men who were found dead in their city apartments. Clothing and devices used showed a great variety. Women's or fetish clothing and complex shackling or hanging devices were disproportionately frequent. In most cases, death occurred due to hanging or ligature strangulation. There was no increased incidence of underlying psychiatric disorders. In most of the deceased no or at least no remarkable alcohol intoxication was found. Occasionally, it may be difficult to reliably differentiate autoerotic accidents, accidents occurring in connection with practices of bondage & discipline, dominance & submission (BDSM) from natural death, suicide or homicide.

Involuntary masturbation and hemiballismus after bilateral anterior cerebral artery infarction.

Ischemia of the areas supplied by the anterior cerebral artery is relatively uncommon. In addition, combined hemiballismus and masturbation have rarely been reported in patients with cerebrovascular disease. We describe herein a 62-year-old right-handed man simultaneously exhibiting right side hemiballismus and involuntary masturbation with the left hand after bilateral infarction of the anterior cerebral artery territory. Right side hemiballismus was related to the disruption of afferent fibers from the left frontal lobe to the left subthalamic nucleus. Involuntary masturbation using the left hand was exclusively linked to a callosal type of alien hand syndrome secondary to infarction of the right side of the anterior corpus callosum. After 2 weeks, these abnormal behaviours were completely extinguished. This report stresses the wide diversity of clinical manifestations observed after infarction of the anterior cerebral artery territory.

Female autoerotic deaths--still often overlooked: a case report.

Female autoerotic deaths are less common than male ones and usually present in a less obvious way. Thus, such deaths can be easily overlooked, even by senior investigators, if the latter are not fully aware of the particularities of female autoerotic cases. We present the case of a 34-year-old woman, found dead in her apartment, with several foreign body insertions and a dog leash around the neck. Despite the great experience of the investigative team, autoerotic death was not considered at first. This case illustrates the importance of forensic teams having knowledge of the particularities of female autoerotic deaths.

Genetic influences on variation in female orgasmic function: a twin study.

Orgasmic dysfunction in females is commonly reported in the general population with little consensus on its aetiology. We performed a classical twin study to explore whether there were observable genetic influences on female orgasmic dysfunction. Adult females from the TwinsUK register were sent a confidential survey including questions on sexual problems. Complete responses to the questions on orgasmic dysfunction were obtained from 4037 women consisting of 683 monozygotic and 714 dizygotic pairs of female twins aged between 19 and 83 years. One in three women (32%) reported never or infrequently achieving orgasm during intercourse, with a corresponding figure of 21% during masturbation. A significant genetic influence was seen with an estimated heritability for difficulty reaching orgasm during intercourse of 34% (95% confidence interval 27-40%) and 45% (95% confidence interval 38-52%) for orgasm during masturbation. These results show that the wide variation in orgasmic dysfunction in females has a genetic basis and cannot be attributed solely to cultural influences. These results should stimulate further research into the biological and perhaps evolutionary processes governing female sexual function.

[Report on a methemoglobin-associated fatality]. / Bericht über einen Methämoglobin-assoziierten Todesfall.

An autoerotic fatality due to mechanical compression of the neck by a sophisticated construction used for breath control games is reported. The presence of signs of methemoglobinemia as well as an empty "poppers" flask found beside the passed indicated the use of volatile nitrites to enhance sexual pleasure. It was assumed that vasodilatation had caused a sudden unconsciousness with consecutive hanging.

Duration of sexual arousal predicts semen parameters for masturbatory ejaculates.

A man's semen parameters may vary considerably from one specimen to the next, partly due to variability in the conditions under which the specimens are produced. In the present study, the relationship between the duration of preejaculatory sexual arousal and the quality of semen produced by masturbation was investigated. Twenty-five regular semen donors aged 22-44 provided a total of 292 semen specimens (median 11 per donor) over a period of 4 months. Each specimen was produced after a minimum of 3 days of ejaculatory abstinence and measures included the time taken to produce the specimen, ejaculate volume, sperm concentration, and percent motility. Linear regression revealed that, controlling for donor identity, there was a significant (t=2.13, P<.05) positive relationship between the time taken to produce a specimen and sperm concentration. We conclude that the duration of preejaculatory sexual arousal is an important predictor of ejaculate quality for specimens produced by masturbation and that variation in the duration of preejaculatory arousal may contribute to within-male fluctuations in semen parameters over time.

[An unusual autoerotic accident: sexual pleasure from peritoneal pain]. / Ein ungewöhnlicher autoerotischer Unfall: sexueller Lustgewinn durch Peritonealschmerz.

The paper describes a fatal autoerotic accident of a 23-year-old man who derived sexual pleasure from painful irritation of the peritoneum by sharp force. For this purpose he clamped two knife blades into a vice suspended from a rope-pulley construction so that he could lower it to pierce the abdominal skin with the tips of the knives. When one of the ropes broke, the vice with the knives fell onto the man's belly piercing the inferior vena cava and leading to death by exsanguination. This, to the authors' knowledge, has been the first ever report of a fatal autoerotic accident in which sexual pleasure was to be obtained from painful irritation of the peritoneum.

A 56 year old man was found suspended in a sack in a head down position. Circumstances and autopsy findings are discussed. The leading pathophysiological mechanisms causing death are thought to be asphyxia due to compression of the chest and insufficient blood supply to the heart due to the head down position. The pathophysiological dysregulation of circulation in a head down position is discussed.
